Returning clients are only asked to fill out a new form if experiencing major health changes or if it has been over a year since last appointment. If you are booking an appointment for another client, please note their name in the "Comments" section after booking and have them fill out an intake form by following the link on Hypnotic Hands website home page. Arriving 5 minutes early is recommended. Late arrivals may not guarantee full appointment time and will be charged for the scheduled amount of time. Always double check your appointment confirmation and appointment reminders to ensure you arrive at your scheduled time. Once your appointment is booked, you will receive a confirmation email at the email provided.
